Dear Mr. Miller,
I have considered the bad conduct that you have shown in your work performance for the past few days. With careful examination, I have decided to give you a one-week suspension that will be effective on March 25, 2010.
I am giving you this last disciplinary notice and I will be expecting changes for your first day of return in our company. You will be force to leave if the bad conduct in your work will happen again.
Please feel free to talk to the Human Resource Department if you have any questions or concerning this disciplinary letter.
